Please call us at (206) 784-9111 to schedule your first appointment at a convenient time and location. We will answer any questions you have and help direct you to one of our exceptional naturopathic physicians or to our registered dietitian nutritionist.

Once you have scheduled your appointment, please download and complete the following applicable forms:

• Download Consent to treat a minor
• Download Telephone consult agreement

All forms must be completely filled out, signed and returned prior to your first appointment.
